1 Have you had a Fracture (Broken Bone) after minor bump or fall ?
2 Have any of your parents had a Fracture after minor bump or fall ?
3 Have any of your parents been diagnosed as Osteoporosis ?
4 Have you lost more than 3 cm in your Height ?
  HISTORY OF PERSONAL HABBITS  
5 Do you regularly drink Alcohol in excess of safe drinking limits ? *
6 Do you smoke ? **
7 Do you have a sedentary lifestyle and no weight bearing exercise ?
  HISTORY OF TREATMENT AND DISEASE  
8 Have you had corticosteroids for more than 3 months ?
9 Have you been bed ridden for a long time due to any reason ?
10 Do you have frequent Diarrhea ?
11 Do have any chronic disease like Rheumatoid diseases like Rheumatoid Arthritis or Lupus, Chest diseases like Asthma or Bronchitis, Hormonal diseases like Hypothyroidism or Cushing's syndrome, Intestinal diseases like Celiac disease or Crohn's disease, that could affect bone building.
